The Chronic Disease Fund is an advocate of Specialty Pharmacy for patients diagnosed with chronic or life-altering disease. These pharmacies maintain specialized expertise and provide significant value-added benefits to patients. Based upon the pharmacy’s expertise and the drug’s special handling requirements, drug manufacturers may limit distribution to selected pharmacies. However, CDF allows patients to fill their prescription at any pharmacy that is able to dispense the product.

Should patients choose to fill their prescription at one of our participating pharmacies the pharmacy will seamlessly work with the Fund to make enrolling in the program as easy and convenient as possible. Receiving the initial prescription and subsequent refills will be even easier. These pharmacies maintain 24/7/365 clinical support by phone should patients have any questions concerning their medication, night or day! Equally important, these pharmacies will provide patients with a kit containing both disease and prescription information along with all of the supplies needed to administer the drug therapy.

For patients choosing to dispense their prescription with one of our participating pharmacies we will pay their out-of-pocket costs directly to the pharmacy alleviating any cash-flow issues. Patients choosing to use any other pharmacy must submit their original receipt to us for reimbursement. A patient’s choice of pharmacy will have no impact on our determination of funding.
