The Chronic Disease Fund has partnered with DiseaseTrak™ to provide therapy management services. Therapy management is an integral part of our program ensuring patients the greatest outcome from their prescription drug therapy.

DiseaseTrak™ provides therapy management through an online patient portal. Upon acceptance into one of our programs, the patient will be required to complete a patient profile and provide us with required additional information. Every month thereafter the patient will complete a short list of questions in order to assist the patient in monitoring their disease progression. This information is contained in a secure environment in compliance with HIPAA guidelines.
