Financial Assistance
Financial assistance is awarded on a calendar year basis to patients that have insurance and meet our income qualification guidelines. Each application for assistance is reviewed by our team taking into consideration income levels by geographic area, the number of household dependents and current out-of-pocket medical expenses. Patients may enroll in our programs either directly through the Fund or through one of our participating pharmacies. Patients are enrolled on a first-come, first-served basis.
The cost to support each patient varies significantly by disease, the prescription drug, and the type of insurance coverage (private insurance or Medicare Part D).
In most circumstances, we do not provide 100% of an individual's out of pocket expenses. We will diligently work with patients to minimize or manage their out -of -pocket costs but we must limit our payment to any single individual in order maximize our donations and help more patients in need.
Therapy Management
Financial assistance is provided in conjunction with therapy management. Our primary objective is to provide financial assistance to assist patients in acquiring prescription medication. Our secondary objective is to assist patients in being fully compliant with their physician’s prescribed treatment plan. These miracle drugs are only effective if taken appropriately. We provide therapy management and disease progression tools at no cost to you to assist you in monitoring your health and achieving your highest quality of life.
Compliance and persistence is the most important part of any therapy management program. Through our partnership with DiseaseTrak™, you will be provided an online patient portal to track your progress in treatment. We ask that you log in to the system once per month to participate in the program.

Required Information
In order to be eligible for our financial assistance program you must complete and sign an Application and HIPAA Authorization. We will review your documentation for approval. However, since we are a charity there is no guarantee that funding will be available. If we are unable to provide you with financial assistance we will refer you to other organizations that may be able to assist you. Please consult our Resources Page for more information.
Programs
Patients with private insurance or a Medicare Part D plan who cannot afford their prescription drug co-payments will have them paid directly by the Fund. Other co-pay assistance programs function on a reimbursement basis, which can cause significant cash-flow problems for most patients. The Chronic Disease Fund has two specific programs that eliminate the financial burden to patients:

Specialty Pharmacy Program:
This program pays the patient’s out-of-pocket costs directly to a participating specialty pharmacy on their behalf. Participating pharmacies work seamlessly with the Fund and provide extra benefits to patients, such as 24-hour clinical support by phone. A patient’s choice of pharmacy has no impact on funding decisions.

Access Card Program:
Patients are provided an Access Card to cover the copayment for their medications. Through this program, the patient can obtain their prescribed medications at the physician’s office, by mail order, through retail outlets or at any specialty pharmacy. They have access to these funds anywhere MasterCard is accepted.
